Home Health Care-Nursing At Home
The Home Health Care Department of CMH provides health services for the homebound patient who is acutely, chronically, or terminally ill; for the patient not necessarily requiring hospitalization, but in need of nursing services. As young as a newborn all the way to the geriatric years, we're here for you. In addition, we also care for pregnant mothers who require bed rest. The purpose is to assist the patient to attain and maintain or restore health and guide the family in supporting and assisting towards this goal. Efforts are also made towards prevention of further disability and on teaching basic health care.
Home Health Care Services Key Responsibilities:
- Skilled Nursing - Assess physical status. Notify physician of changes. Prompt treatment to prevent hospitalization. Perform nursing procedures and coordinate all services. Instruct patient and family to care for patient.
- Physical Therapy - Restore physical functioning for increased independence. Develop home exercise program.
- Speech Therapy - Instruct on swallowing and speech disorders. Develop home exercise program.
- Medical Social Service - Assist patient and family to find community resources. Enable patient to remain in home. Counsel on social and emotional problems.
- Home Health Aide Services - Provide personal care and hygiene, assist and teach activities of daily living to prevent institutionalization.
MEDICAL SUPERVISION is provided by the patient’s physician in consultation with Home Health Services team. Nursing care and treatments are provided with a physician - authorized plan for the patient and family.
